The Northern Kentucky Area Development District announced Wednesday that it is launching a new veterans care program.
Dubbed the Veterans Directed Care program, it is “a person-centered initiative that empowers eligible veterans to remain independent in their homes and communities while receiving the services and supports they need,” according to the development district.
The district will provide case management to eligible veterans who are at-risk of entering nursing home care. Case managers work with program participants to provide a flexible budget and support services to develop a personalized care plan with care givers.
“Eligible veterans can use their individualized budgets to access a wide range of support, including personal care assistance, homemaker services, respite care, transportation and other community-based services designed to help them remain safe at home,” according to the development district.
